When complex healthcare needs arise, our UHA nurses are here to help manage your care.
Our Registered Nurse (RN) Care Specialists, formerly called Care Management Nurses, will work with you and your health care team to facilitate the care you need.
Our RN Care Specialists work with you, your family, and your physician to develop an individualized plan of care during recovery, help with transitions, teach you care skills, and provide counseling – with no additional cost to you.
Eligible members include those with:
- Complex medical care requirements*
- Questions about care or diagnosis and how to access UHA Live Well and Be Well program benefits like diabetes or asthma management classes
- Concerns about expenses incurred from a chronic/complex health condition
- Transition planning from another health plan or to a new health plan
- Hospital discharge and transition planning to home
A RN Care Specialist will contact eligible members, their family and the primary and/or specialty physician. Together, (RN Care Specialist, member, member’s family and provider) we will develop a plan of care and goals.
Additionally, we can help to connect you with our great benefits and programs to meet your specific health needs.

RN Care Specialists Improve Health Outcomes
Our experience tells us that the best health outcomes occur more often when there’s a little more TLC involved.
- Shorter hospital stays. The RN Care Specialists will help to ensure that you have the care you need when you return home.
- Lower risk of complications. The rate of hospital-induced complications such as infection or blood clots are reduced.
- Better managed benefits. The RN Care Specialists can often lower costs by managing your benefits in-home or at the hospital.
- Faster recovery. We’ll help get you back on your feet and back to work.

Concurrent Care
If you should become seriously ill, your plan also includes concurrent care benefits so that you receive the extra support of hospice while continuing your treatment.
Concurrent Care When You Need It
Concurrent care means that somebody with a serious illness can receive comfort care and still be covered for life-prolonging treatments by a team of healthcare experts. Your UHA plan covers this.
