Becoming a UHA participating provider has its rewards. UHA is Hawaii's only health plan founded and guided by physicians.

    Our Approval Criteria

    Providers in our network are selected and approved based on the review of Provider’s applicable certifications and licensing, submission of completed applications and other documents, network availability, and approval by UHA’s Chief Medical Officer.

    Disclaimer: UHA’s healthcare network is within the State of Hawaii only. We participate with UnitedHealthcare’s Options PPO Network to provide healthcare services to our members when traveling outside of Hawaii. Insurance coverage is provided by UHA. The administrative services are provided by United HealthCare Services, Inc.

    About our application process

    New providers or those already part of a participating office who need to be added must complete the Provider Enrollment Application to become a participating UHA provider.

    Once you’ve submitted your application, please allow 4-6 weeks before contacting us regarding the status of your application.

    Follow these steps to get started:

    1. Review our list of Professional Credentialing Requirements.
      Find your specialty in the above list and review the credentials and/or documentation we require.
    2. Get your documents ready.
      We ask all specialties to provide these documents with their application:
      (Accepted file formats: PDF, JPEG, GIF, BMP, TIFF, PNG)
      • CV
      • License
      • Board Certification
      • W-9
      • DEA (if applicable)
    3. Complete the Provider Enrollment Application.

      We recommend setting aside at least 20 minutes to complete the application. Need more time? Use the “Save for later” button in the lower right-hand corner of the application screen to generate a “Resume code.” Save this code and re-enter it on the form later to pick up where you left off.

    4. Give us 4-6 weeks to review your application.

      If you’re already a participating provider, you can log in and check your application status under the “Form Status” tab in your Online Provider Services provider portal account.
