Important Notice for Members: Race & Ethnicity Demographic Data Survey

UHA is asking for your assistance in completing the voluntary Race and Ethnicity Demographic Data Survey. This survey is a federal initiative by the U.S. Department of Health and Human Services (HHS). Your voluntary answers, which will be provided to HHS, are aimed at gathering race and ethnicity data to identify potential disparities in healthcare and support a broader understanding of the health needs and gaps in coverage across the United States.

In collecting race and ethnicity data, UHA Health Insurance will:

1. Provide the data to HHS and maintain all the data as private within UHA.
2. Not use the data for eligibility determination, underwriting, or rating purposes.
3. Not deny your application based on whether you choose to answer these questions.

We kindly request you to complete this survey for you and your family members by Friday, February 28, 2025. If we do not receive your completed survey by the deadline, we will assume that you are choosing to not participate and report your response as “Decline to Report” in our data submission to HHS.

How to Complete the Survey:

1. Log in to your UHA Online Member Services account here.
2. When prompted, please provide the requested information.
