Both of our Preferred Provider plans (PPO) are designed to meet ACA requirements and are bundled with prescription and vision to make it easy to choose the right plan.
Plans differ by co-pay amounts and your total out-of-pocket costs, as shown in the chart below. If you're younger and healthier, our standard plan may be the one for you. Both plans make it easier for your doctor to provide the medical care that's right for you.
Benefits & Programs

A dental program can be added to either plan.

Complementary Alternative Medicine Benefits
UHA will pay up to $500 a year* for chiropractor or acupuncture visits. Choose a provider from our online directory to qualify for a $10 co-payment per visit. We'll also pay 50% of the eligible charge for the first set of x-rays ordered by a participating chiropractor.Find a participating provider
