Explore Our Plans
Both of our Preferred Provider plans (PPO) are designed to meet ACA requirements and are bundled with prescription and vision to make it easy to choose the right plan.
Plans differ by co-pay amounts and your total out-of-pocket costs, as shown in the chart below. If you're younger and healthier, our standard plan may be the one for you. Both plans make it easier for your doctor to provide the medical care that's right for you.
Benefits & Programs
At UHA we want you to live well, be well, and work well, so each plan includes a full suite of benefits and programs like our Live Well member lifestyle programs and Be Well benefits for prevention and chronic condition management.
A dental program can be added to either plan.
Hawaii Dental Service is the largest nonprofit dental service corporation in Hawaii. More than 95% of all licensed, practicing dentists in Hawaii participate with HDS, creating the largest network of dentists in the state.
HDS is part of Delta Dental, the largest dental insurer in the nation, and that means you can receive dental coverage from participating dentists when you and your family are on the Mainland for school, work or vacation.Find an HDS dentist
We offer two comprehensive fitness plans.
As an employer, you can decide whether to subsidize your employees' fitness center memberships or not. UHA offers Active and Fit and Fit Flex to help your employees enjoy the best fitness options in the state for minimal fees.
